The OpportunityKia mau kit e tokanga nui-a-noho - There is no place like home - for people with specialist palliative care needs and their families this whakataukī is all too true.Our goal is to increase our specialist services to reduce unnecessary hospital admissions and ensure people can have the level of care they need to stay at home.The Korowai Care programme provides intensive support to people with specialist palliative care needs in their own home, to manage difficult symptoms or end of life care for a short period. This will be as an alternative to an inpatient unit stay when people would prefer to be at home.As the Clinical Nurse Lead you will lead the delivery and further development of this programme of care. You will work with a small team of health care assistants and casual staff to provide the enhanced level of assessment and care. Our wider professional inter-disciplinary Community Palliative Care team will work alongside you to deliver specialist palliative care to the West Auckland Community.The role is full time to be worked Monday to Friday. As this is a clinical leadership role, it will require some flexibility in responding to service needs.About YouAs a Clinical Nurse Specialist (CNS) or Nurse Practitioner (NP) in Palliative Care you have a passion and interest in holistic patient centred care. You will love working with diverse people and communities and have strong leadership and organisational skills.You will need to be:
  • A CNS or NP with palliative care and community experience
  • Hold a post graduate nursing qualification (palliative care desirable but not essential)
  • Current registration as a New Zealand Registered Nurse or NP
  • Minimum of 5 years' experience post registration
  • Previous experience in a nurse leadership role an advantage
  • Able to effectively work as part of a multidisciplinary team
  • Knowledge of the Treaty of Waitangi with a commitment to Biculturalism
